Since 2010, Wendy McEntyre has submitted repeated public records requests to the California Department of Health Care Services (DHCS), seeking full disclosure of facility-related deaths. Her goal: to create an easily searchable, transparent database documenting deaths in licensed addiction treatment facilities and sober living homes.
